what if paper towel was left in DISA hole???

I think I might have left a paper towel in the DISA hole that I put there to make sure nothing else went in. After cleaning the ICV and removing the Intake manifold and Disa. I started up my car (which ran previously) and it started for about 1-2 seconds then got rough for a second and then died out. Would not start again. I don't think I did but its possible I left a paper towel in the hole and maybe pushed it in when the Disa was reinstalled. What would this have caused if it was left in there and what would need to be taken apart? I did try to look into the hole and saw nothing. Any idea what could cause this? Its also possible I left a paper towel in the throttle body on the outside of the trap door. Nothing was there though when I took the car apart, maybe sucked in? Car was only on idle for 2 seconds before shuttering and dying a second or 2 later. Any ideas or suggestions welcomed. The code was for fuel shut off. Fuel pump was 1-2 years old but only 3,000 miles old, gas tank at 1/4th after filling with some gas
